Chronic Disease Management Plans

At Medsana Medical Clinic we take a team approach to managing chronic disease. Our doctors, specialists, nurses and allied health professionals together, create a management plan that is tailored to your specific needs.

Our doctors regularly attend education sessions to ensure they are using the latest evidence-based guidelines and up-to-date research in their clinical practice. We have a particular interest in childhood asthma, allergies and eczema.

We have the latest in diagnostic medical equipment to help in the management of asthma and cardiovascular disease. We perform lung function testing and ECGs on site.

Asthma

Asthma is a medical condition that affects the airways.  From time to time, people with asthma have difficulties breathing because the airways to their lungs become narrower.  At other times their breathing is normal.

Symptoms of asthma may include:

  • wheezing
  • a feeling of not being able to get enough air or being short of breath
  • a feeling of tightness in the chest
  • coughing

You don’t have to have all these symptoms to have asthma.

Diabetes

Diabetes is a serious complex condition which can affect the entire body. Diabetes requires daily self-care and if complications develop, diabetes can have a significant impact on quality of life and can reduce life expectancy. While there is currently no cure for diabetes, you can live an enjoyable life by learning about the condition and effectively managing it.

There are different types of diabetes; all types are complex and serious.

Our health professionals will work as a team to optimise your diabetes management.  We recommend regular 3 monthly reviews to help keep your diabetes on track and reduce the risk of hospital admissions.

Allergies/intolerances

Allergy occurs when a person’s reacts to substances in the environment that are harmless to most people. These substances are known as allergens and are found in dust mites, pets, pollen, insects, ticks, moulds, foods and some medications.

Atopy is the genetic tendency to develop allergic diseases. When atopic people are exposed to allergens they can develop an immune reaction that leads to allergic inflammation. This can cause symptoms in the:

  • Nose and/or eyes, resulting in allergic rhinitis (hay fever) and/or conjunctivitis.
  • Skin resulting in eczema, or hives (urticaria).
  • Lungs resulting in asthma.

Medsana Medical Clinic will collaborate with your allergist to manage your allergic condition.  We can give subsequent maintenance desensitisation injections according to your schedule.

We want to keep your heart healthy.  Our team at Medsana Medical Clinic will work with you to be smoke-free, manage your blood cholesterol, manage your blood pressure, be physically active, achieve and maintain a healthy weight, enjoy a variety of nutritious foods and look after your mental health.

Our GP care may involve the dietitian, exercise physiologist, physiotherapist, psychologist, cardiologist and interventionist cardiologist to name some.

 

Osteoporosis

What are some of the things you can do to improve and maintain your bone health? 

They include:

  • adequate calcium levels,
  • adequate Vit D levels,
  • avoiding negative lifestyle factors such as smoking and excessive alcohol,
  • Bone friendly exercise,
  • Taking medicine as directed if you have been diagnosed with osteoporosis.
